Consent form template for photos, studies, trips and services
Parents need to approve a school trip, interviewees need to agree to be recorded, or clients need to OK a photo shoot or a beauty treatment. This prompt has your todo.is agent write a clear consent form for your exact situation: what people agree to, what is optional, how to withdraw, and where to sign.
The prompt
- Create a consent form for [WHAT PEOPLE ARE CONSENTING TO], used by [ORGANIZATION OR PERSON]. The people signing are [WHO SIGNS]. Explain in plain language: what will happen, why, any risks or discomfort, what data, photos or recordings will be collected, how they will be used and stored, who sees them, how long they are kept, that taking part is voluntary, and how to withdraw consent at any time by contacting [CONTACT DETAILS]. Add separate checkboxes for each optional item: [OPTIONAL ITEMS]. End with name, signature and date lines, and a parent or guardian section if needed. Keep it to one page if possible, reading level of a 12-year-old. Save it as Word and PDF, and give me a short version I can send as a message.
What to change
- [WHAT PEOPLE ARE CONSENTING TO]: e.g. "a 45-minute recorded interview for a university study on remote work", "a school trip to the science museum", "photos at our studio".
- [ORGANIZATION OR PERSON]: Who is asking, e.g. "Riverside Primary School" or "Dr. Ana Kovač, Westbrook University".
- [WHO SIGNS]: e.g. "adult participants", "parents of students aged 8 to 10", "salon clients".
- [CONTACT DETAILS]: Name, email or phone people use to ask questions or withdraw.
- [OPTIONAL ITEMS]: Things people can say yes or no to separately, e.g. "audio recording, quotes in the report, photos on social media".

How to do it with todo.is
- Copy the prompt and describe what people agree to, who signs and the optional items.
- Paste it into todo.is on the Today screen or send it to your agent on Telegram.
- Your agent writes the form and sends a Word file, a PDF and a short message version.
- Ask for a version in another language or a parent section, then print or share it.
Tips for a better result
- Give each optional item its own checkbox. Bundling "photos, recordings and marketing" into one yes is not real consent.
- Say how to withdraw and who to contact. People trust forms that make saying no easy.
- Write for a 12-year-old reading level. Short sentences make consent valid in practice, not only on paper.
- Ask your agent to translate the form so families or participants can read it in their first language.
- Medical, research and school forms often follow an official template or ethics board rules. Check those first. This draft is not legal advice.
consent form template: FAQ
- What should a consent form include? What will happen and why, risks, what data is collected and how it is used and stored, that taking part is voluntary, how to withdraw, contact details, and signature and date lines.
- What is informed consent? Informed consent means the person understands what they are agreeing to, including risks and alternatives, and agrees freely. The form records that this explanation happened.
- Is a signed photo consent form needed for social media? It is good practice and often required, especially for children and in places with privacy laws like GDPR. Keep photo consent separate from other consents.
- Can I use this for medical procedures? Clinics usually need forms that meet their regulator's or insurer's rules. Use the draft as a starting point only and have it checked by your professional body or a lawyer.
